Before Your First Visit
Important Information

Regarding antibiotic premedication for dental treatment:

Please call our office before the day of your visit if you have been advised by your physician or dentist to use an antibiotic medication before dental procedures because you have one or more of the following conditions: prosthetic heart valve, previous history of infective endocarditis, congenital heart disease (unrepaired heart defects or heart defects completely repaired with prosthetic material or device within 6 months), repaired congenital heart disease (CHD) with residual defects, heart transplant, prosthetic joints (hips, knees, shoulders, etc).

Please bring a list of all medications you are currently taking.

All patients under the age of 18 must be accompanied by a parent or legal guardian.
